• Please open the "Fillable pdf" file named "Game Sheet and Team Discipline Log" (below). If you cannot open the file, you probably need a newer version of Adobe Reader. To get a free copy of this program, click on the red square beside the documents' titles. This link will take you to the Adobe site, where you can download the latest version of Adobe Reader.
  Although the document that you will see does not look like a Game Sheet or a Team Discipline Log, simply fill in the required and requested information.
  Once all of the info has been entered and you are ready to print the documents,PRESS THE PRINT BUTTON AT THE BOTTOM OF THE PAGE THAT YOU HAVE JUST FILLED IN. The program will automatically insert the inputted information into the appropriate fields of the Game Sheet and the Team Discipline Log. The program will automatically print ten (10) Game Sheets and one (1) Team Discipline Log.
  Do NOT press "Control-P" or a printer icon because pushing those buttons will cause you to print just the page on the screen which is neither a Game Sheet nor a Team Discipline Log.
• Please download and complete one (1) copy of the Bench Staff List and submit it when you register your team.
• A Travel Permit is required by ALL out-of-district teams. Basically, if your players' books contain a district stamp other from than the EODSA, you require a Travel Permit - even if you play in the ERSL. Please FAX the Travel Permit in advance to 613-237-1890. Quebec teams: Please note that you need TWO (2) stamps of approval on your Travel Permits.
• Registration will be held at the Jim Durrell Centre on Walkley Road (about 400 yards east of Bank Street) from 6:00 pm until 9:00 pm on Thursday May 27 (for the Girls' Icebreaker) and on June 3 for the Boys' Icebreaker).
• At Registration, you will need to have your player cards (or books) properly completed and approved by your District Association. You will also need to show the Official OSA Team Roster for your team AND FOR THE USUAL OUTDOOR TEAM FOR EACH GUEST PLAYER. Please note that the OSA Roster DOES NOT replace any player's missing card or book. You must have the cards or books. Although we will NOT keep a copy of the Official Roster, you will need to keep it on hand in case any player eligibility questions (etc) arise during the tournament.
• Please download, read and keep on hand a copy of the tournament rules (Long Version).
